For your convenience, we have enclosed printable registration and financial policy forms which can be completed at home. This will eliminate the need to arrive early to complete this paperwork and allow time for you to find physician phone numbers, medication names, etc. In addition, you will need a photo ID and proof of insurance coverage at your first appointment. We will make copies of these items and place them in your chart. They are kept confidential as are all sections of your records.
